whilst I am in

perfecte memory

(praised by God)

my last will and
Testament in
manner and forme
following (that
is to say) First
and principally
I Commend my
Soule into the
hands of
Almighty God my
Maker trusting
assuredly for
Salvation by the
alone meritts of
my blessed Lord
and Saviour
Jesus and my
Body I Commend
to the Earth

came in assured
hope of Joyfull
and Glorious

to bee buried
